I-9 Form for New Employees

Summary

All employees must submit proof of identification in person for the form I-9 by their first day of work. This article provides a link to the list of acceptable documents as well as information for remote or out-of-town employees who are unable to submit their identification in person.

Overview - Form I-9

All employees must submit proof of identification in person for the form I-9 by their first day of work. Identification must be current and original - copies, photos, or expired identification cannot be accepted. 

Employees must complete section 1 of the form in Workday before providing their identification to a team member from the Office of Human Resources - Workday will prompt you to do this as part of your onboarding tasks.

Out-of-Town or Remote Employees

If you are unable to present your identification in person on your first day of work, you can have it notarized and submit copies of your identification instead. Please print the document attached here and bring it with your identification to a local notary. You must bring the original identification to the notary to complete this form.

Once the form is complete, the notary will make a copy of your identification and must send both the copies as well as the signed and notarized form to:
