Overview
Lock and key changes are provided by Facilities Management for all University buildings. Only Facilities Management employees are authorized to change locks or to make copies of keys to University buildings.
Information Required
Upon submitting the key request form, your supervisor will be prompted with an approval step. That step must take place before the key is issued. Departments will be charged for lost keys, lock changes, and extra locks.
The cost of a replacement key is $80. Additional charges for new cores or door hardware resulting from lost keys will be quoted after ticket review.

Please submit one key request per key holder.
