What is it?
Submit this form to request to
- change your primary major and/or concentration
- add or drop a secondary major
- add or drop a certificate
- add or drop a minor
You must submit a separate request for each change or addition. For example, if changing your primary major and requesting your primary major become your secondary major, you would submit a primary change of major request AND an add a secondary major request.
